Life Alert Costs and Contracts 2026

Medical Alert Pendant

Everyone knows Life Alert. The commercials have been running since the 1980s, and “Help! I’ve fallen!” is probably the most famous medical alert tagline ever. But brand recognition and actual performance are two very different things, and we need to be honest with you about what we found when we tested Life Alert ourselves.

We ordered the full Life Alert bundle — home system, wall button, mobile pendant — and put it through the same testing we do for every company. The results were disappointing. It took them an average of 120 seconds to answer our test calls. Two full minutes. Bay Alarm Medical answers in 16 seconds. And Life Alert charges nearly double what most competitors charge, locks you into a 3-year contract, and doesn’t offer fall detection at all.

We don’t say this lightly, but we think most families will be better served by almost any other company on our site. Here’s the full cost breakdown so you can see exactly what you’re signing up for.

Editor’s Rating: 1.8 / 5

Life Alert at a Glance

Life Alert starts at $49.95/month — nearly double the starting price of Bay Alarm Medical ($24.95/month) and significantly more than Medical Guardian ($31.95/month). All plans require a 3-year contract.

System Costs Breakdown

SystemMonthly CostOne-Time FeeNotes
Micro Voice Pendant (home)$49.95$95Required base system

On-the-Go + GPS (mobile)+$20$198*Add-on only

*Activation fee of $198 applies to bundled systems. The $95 activation applies to the home system alone.

Neither the wall-mounted button nor the mobile GPS pendant can be purchased independently. Both must be added to the $49.95/month home system.

Life Alert Systems

Micro Voice Pendant System (Home)

The base home system includes one cellular base unit and one wearable help button (worn as a pendant or bracelet). Pressing the pendant initiates a call through the base unit’s speaker. The base unit has a built-in 72-hour backup battery for power outage protection.

Help button range: 500 feet from base unit — shorter than Bay Alarm Medical (1,000 feet) and MobileHelp (1,400 feet).

HELP Button (Wall-Mounted)

A wall-mounted button with its own cellular connection, speaker, and microphone. Unlike the wearable pendant, this device operates independently of the base unit. It can be mounted in a shower or bathroom (IP67 rated), making it useful for the location where most falls occur.

Battery life: up to 10 years. Must be purchased as an add-on to the home system ($20/month).

On-the-Go + GPS (Mobile Pendant)

A mobile device with GPS and a built-in speaker. Provides protection outside the home. In testing, agents accurately reported location during mobile calls.

Battery life: up to 10 years (disposable, not rechargeable). Must be bundled with the home system ($20/month add-on).

Life Alert Contracts

Life Alert’s contract terms are the most restrictive in the medical alert industry:

  • Minimum term: 3 years
  • Early exit conditions: Death of the user or permanent move to a 24/7 care facility
  • No trial period: No documented 30-day return window in the service agreement
  • No fall detection: Standard feature at most competitors; not available with Life Alert

For comparison: Bay Alarm Medical, Medical Guardian, MobileHelp, and Lively all offer month-to-month service with no long-term commitment.

What Life Alert Is Missing

Life Alert devices work reliably — the base unit’s speaker is loud, help buttons are lightweight, and the GPS mobile pendant tracked accurately in testing. What Life Alert lacks compared to modern competitors:

  • Automatic fall detection — absent entirely
  • Caregiver app — no location tracking or emergency notifications for family members
  • Competitive response times — tested at 120-second average (competitors average 8–26 seconds)
  • Flexible contracts — 3-year commitment vs. month-to-month everywhere else

The One Genuine Advantage

Life Alert’s help buttons and mobile pendant use disposable batteries rated for up to 10 years. For users who struggle with device charging or whose loved ones are unlikely to remember daily charging, this long-lasting battery design is a practical benefit not available elsewhere.

This single advantage does not justify the price premium or contract terms for most users. But it is worth noting for caregivers whose primary concern is ensuring the device is always powered.

Bottom Line

Life Alert costs more, requires a longer commitment, and performed worse in response testing than every competitor we evaluated. The 3-year contract with limited exit options makes it especially difficult to recommend for anyone who might want to switch providers or cancel service.

For the rare case where battery life is the overriding concern, Life Alert’s 10-year disposable battery is a genuine differentiator. For everyone else, Bay Alarm Medical, Medical Guardian, or Lively offer better protection at lower cost with no long-term commitment.

Prices and availability should be verified on the provider’s website.

Frequently Asked Questions

How much does Life Alert really cost per month?
Life Alert's home system (Micro Voice Pendant) costs $49.95 per month plus a one-time $95 activation fee. Adding the GPS mobile pendant or wall-mounted help button each costs an additional $20/month, but neither can be purchased without the home system. A full bundle with all three devices costs approximately $89.95/month with a $198 activation fee.
Does Life Alert require a contract?
Yes. Life Alert requires a 3-year service agreement. The contract can only be terminated early if the user passes away or permanently transitions to a 24/7 care facility. No other exceptions apply, making Life Alert one of the most restrictive contract structures in the medical alert industry.
Can I cancel Life Alert after 30 days?
Life Alert does not offer a standard trial period or 30-day cancellation window comparable to competitors. There is reportedly a brief window of a few business days after receiving equipment when you can return it, but this is not documented in the service agreement. After that window, you are bound to the full 3-year term.
Does Life Alert have fall detection?
No. As of 2026, Life Alert does not offer automatic fall detection on any of its devices. Users must manually press a help button to trigger a call. This is a significant gap compared to most medical alert competitors, which offer fall detection as an add-on or built-in feature.
Is Life Alert covered by Medicare?
No. Life Alert and other medical alert systems are not classified as durable medical equipment and are not covered by original Medicare. Some Medicare Advantage plans and supplemental private insurance policies offer discounts or partial coverage. Contact your insurer directly to check eligibility.
